Please start by reading the Welcome letter first, as it provides essential information regarding how our clinic functions and will answer many of your questions. You may also wish to print this out for your own reference.
Please do not wear any strong fragrances when you visit the clinic. Many of our patients and staff are highly sensitive to chemicals so we reserve the right to reschedule your appointment if you are wearing a strong fragrance, lotion, or essential oil. We don’t mean to offend you with this policy, we are just trying to create a safer space for everyone. Thank you for understanding.
Please print and fill out the Health History Form and Informed Consent and bring them with you to your first appointment. Community Acupuncture depends upon your support. By helping us in this way, we have more time to focus on your health as opposed to shuffling papers. We are grateful for your participation in community acupuncture.
If you don’t have the time or ink to print these out and bring them with you, please arrive 10-15 minutes early to your first appointment and you can fill them out in our waiting room.
